What MAPP is

MAPP, or Mobilizing for Action through Planning and Partnerships, is a assessment tool designed to improve health and quality of life through community-driven strategic planning.  Through MAPP communities improve health through identifying and using resource well by forming partnerships with respect to their community. 

This tool was developed by NACCHO (National Association of County & City Health Officials) and has been used in communities national wide to create a healthier, more connected home for its citizens.

Examples:

Richland County - A three year assessment lead to several action groups putting together goals for improvement.  The assessment lead to discounts for senior citizens and full memberships through certain insurance providers.  To encourage exercise in school aged children, safe walking paths to schools were built.
Prairie County - Identified 9 areas that needed improvement, such as access to health care and quality of life.  Barriers to health care included high cost, no local choices, no providers, and distrust of the health care system.  Since the assessment, completed in July 2011, a Community Health Improvement Plan is being constructed.
